MAKING CONNECTIONS is a volunteer led charity with a mission to
support older people experiencing loneliness or isolation: people who
wish to live in their own home, stay connected to their local
community and avoid premature admission to long term residential care.
We work with a number of partnerships including HSE clinicians, to
provide frontline services to people over **Apply on the website**yrs
through support coordination, community meals, befriending and
wellbeing supports.
We are now recruiting a full-time Support Coordinator. The successful
candidate will work closely with staff and volunteers to enhance the
service delivery and activities of the charity and meet the targets
set out in the work plan.

ROLE OVERVIEW
The successful candidate will work with staff and volunteers to
enhance the service delivery and activities of the charity and meet
the targets set out in the work plan.
The Support Coordinator will be responsible for managing a
comprehensive caseload, working closely with partners and coordinating
services to enable and empower older people to age well at home.
A full clean driving licence and use of a car is essential for this
role
RESPONSIBILITIES
* Promote Making Connections Befriending and Support services
throughout the community
* Work closely with HSE primary care teams, discharge planning
nurses, local age friendly partners and other relevant agencies within
the Dublin South
* Manage a caseload of older persons and carry out a comprehensive
needs and risk assessment - in person or by phone and manage the
response to ensure their needs are identified and supported
* Manage, review and assess effectiveness of support plans and
coordinate supports across various groups to ensure delivery
* Develop and maintain strong referral relationships with statutory
and non-statutory agencies
* Recruit, garda vet, train, match and manage volunteers to carry
out the Befriending and Supports role effectively
* Ensure all newly trained and vetted volunteers support older
persons within the specified timeframe
* Identify local activities and events that older persons and
volunteers can participate in
* Provide regular and ad-hoc accurate reporting for Management, HSE
and funders
* Maintain confidentiality and discretion relating to older people,
volunteers and partner organisations
* Review and maintain policies and procedures including
safeguarding, insurance, data protection, health and safety etc.
* Your role will also involve providing advice reassurance and up to

Government/HSE guidelines
* Undertake any other duties of a similar level and responsibility
that may be required
PERSON SPECIFICATION
* ESSENTIAL: Background/experience in one or more of the following
areas; community development, social care, older persons services,
health and wellbeing, volunteer management
* DESIRABLE: Third level qualification in the field of healthcare,
community development, health promotion or equivalent
* A self-motivated person committed to the mission of Making
Connections
* Excellent interpersonal skills, with an ability to network and
negotiate effectively with individuals, groups and stakeholders
* Experience in person-centred care planning, service delivery or
service coordination in the private, public or voluntary sector
* Experience of working in a multi-agency or multi-stakeholder
environment
* Experience in problem solving and excellent project management
skills
* Well-developed skills and aptitudes in forging partnerships and
relationship building
* Solution focused person with excellent organizational and report
writing skills
* A person with strong attention to detail and commitment to deliver
a high-quality service
* Experience of/knowledge and understanding of the local Age
Friendly programme and local health & community services is
advantageous
* Ability to maintain a high standard of confidentiality and
discretion
* Excellent IT skills including Microsoft Office and familiarity
with CRM databases desirable
